WHAT IS A COMMUNITY PLAN?
A community plan sets the goal of preventing, reducing and ending youth homelessness using the Collective Impact framework.
Through community engagement, partnerships and collaboration, communities can harness local and public resources. They can build on existing knowledge about ‘what works and for whom?’, adapt and implement innovative and evidence based program models that are known to work.
A community plan to prevent and end youth homelessness outlines a comprehensive strategy to identify what needs to be done to shift the focus from managing the problem to a more solutions and outcomes based approach.
The overall goal of the plan is to:
- Minimize the risk of homelessness by providing supports to young people and their families
- Help young people reach their full potential by providing the supports and opportunities necessary to grow into adulthood in a safe and planned way
